How to check your insurance coverage:
Call member services (number on the back of your card) and ask:
Does my plan cover medical nutrition therapy/nutrition counseling?
If yes, how many how many sessions are allowed?
Does my plan require an authorization number? If so, your PCP needs to call your insurance company to submit the referral and obtain an authorization number. We must have that authorization number prior to your appointment in order to submit billing.
Is this benefit limited to specific diagnoses?
Do I have a deductible to meet first?
Do I need a physician referral? (If yes, this must be done 3 days prior to your visit)
Do I have a co-pay?
